The scene in Indy was technically not a blooper, it was proposed to the director by Ford because of the food poisoning and Spielberg liked it, so they did it.
Also Joker blowing up the hospital was a blooper that made the final cut. Yes, it was in the script, but during shooting, the pyrotechnics malfunctioned and Heath Ledger decided to fiddle with the detonator, while series of detonations followed afterwards, which scared Heath.

It wasn’t an improvisation, they discussed all the changes to the scene before filming.

He was the Joker.
Ledger was amazing in that movie and its really unfortunate how he died to drugs after. He was by far the best Joker
its crazy how much heath ledger improvised in dark knight. He made the movie tenx better by not following the script all the time.
I always used to laugh how he just pulled up a gun and shot him after he showed off his sword skills
